A) Copy of State of Illinois Police Certification (if applicable) (As prescribed by the Illinois Law Enforcement Training and Standards Board if applicable)
B) Copy of Birth Certificate
C) Copy of Valid Drivers License
D) Copy of Military Discharge Papers and Complete DD-214 (if applicable)
E) Copy of Valid IL FOID Card (or copy of notice of application for FOID)
F) Copies of High School and College Education Diplomas and/or a Valid G.E.D. Certificate
G) Copies of all Police Training Certification (if applicable) (IE: Youth Officer, Breathalyzer)
H) Any letters of recommendation and/or commendations (if applicable)
I) Signed Waiver (Financial Information) – SEE BELOW
J) Current 3×5 Color Photograph (image of applicant only)
K) If a Naturalized Citizen attach Copy of Certification (if applicable)
L) Signed Fact Sheet (retain a copy for your reference) – SEE BELOW
